How can I add or remove someone from my policy?

Who is this for?

If you're a Blue Cross Blue Shield of Michigan or Blue Care Network member, this information will show you how to add or remove someone from your policy.

Get married recently? Have a baby? If you’re experiencing some changes in life, you may need to change who's listed on your Blue Cross Blue Shield of Michigan or Blue Care Network policy.

What do I have to do?

You’ll need to fill out a Change of Status form. It’s part of a group of forms. If you only want to add someone to or take someone off your policy, just fill out page 6 of the document. There are instructions on page 7.

Please send the form within 30 days of the event that changes who’s on your policy.
