As an Blue Care Network (HMO) plan member, you'll need to schedule your annual health exam with your primary care provider. This is a covered benefit of your plan.
If you haven’t chosen your primary care physician yet, please use Find a Doctor to search for a doctor that best fits your needs.
Preparing for your appointment
As you get closer to your appointment, take some notes about what you’d like to discuss with your doctor. You’ll also want to be prepared to discuss your medical history and any special needs you may have.
In addition, your doctor will want to know about any care you’re getting from a specialist so that you can continue to receive this care.
