How do I apply for the HCTC?

If you’re ready to get started, please fill out this new enrollment/change of status form (PDF) to apply for our HCTC Plan 1 or Plan 2. You can find instructions to help you fill out the form on Page 3 of the PDF.

You'll also find additional instructions about applying for the HCTC plans in our cover letter (PDF).
