Michigan Health Insurance – Customer Service – HMO Plans

This information explains what Blue Care Network's two wellness-based health benefit plans have in common, and how they're different.

Both plans come with the rewards of better health and lower health care costs. Here's what makes them different.

Enhanced: maximum benefit level with lower deductibles and copays. Members and covered spouses* need to be actively working towards meeting the plan's health targets. Enhanced: maximum benefit level with lowest deductibles and copays. Members and covered spouses* must meet the plan's health targets.
  Intermediate: average level of deductibles and copays. Members and covered spouses need to be actively working towards meeting the plan's health targets.
