How can I apply?

If you’d like coverage under our BCN 65 plan, please fill out an application. Enrollment applications are accepted year-round. Your application will be approved if you pass our insurance guidelines. You’ll just have to: 

• Download, print, complete and sign the BCN 65 Enrollment Form (PDF). 
• Mail the completed enrollment form to:
