Am I eligible?

Personal Blue Dental℠ plans work with your medical health care plan to give you all-around coverage.

You’re eligible for a Personal Blue Dental plan if you:

• Live in Michigan at least six months of the year
• Are enrolled in one of the following health care plans:
  • Young Adult Blue Max℠
    
  • Keep Fit℠
  • Smart Select™
  • OneBlue℠

You’re not eligible for a Personal Blue Dental plan if you have a Flexible Blue II℠ or Individual Care Blue Plus℠ plan. You’ll need a Flexible Blue Dental℠ plan if you have one of those plans.
