The EOB shows:

  • Services we pay
  • Whether you owe any out-of-pocket costs, like deductibles and copayments
  • Charges we can't pay and why

If you did not receive the services, or if you find discrepancies or have other questions, please call the customer service number on the back of your ID card or the phone number on your EOB.
