PPO
Our preferred provider organization plans establish provider networks that encourage you to stay in-network to avoid additional out-of-pocket costs. Our PPO network is the largest in the state and includes every Michigan hospital.
Community Blue PPO
Hospital, physician services and preventive care are covered under this popular PPO plan. Community Blue is included in the national BlueCard program so you can receive care outside of Michigan. When you receive covered services from participating providers you will only have to pay for your deductible and copayment.
Flexible Blue
Our new health savings account-compatible product for the consumer-directed health market, Flexible Blue combines a PPO plan with a personal savings account. You use money in the health savings account, health reimbursement arrangement or flexible spending account (depending on which form of funding your employer selects) to pay for noncovered services. HMO
Blue Care Network plans emphasize prevention of disease and illness as a way of reducing costs and helping you optimize your health. You select a primary care physician from the more than 3,700 health care providers in the network. The PCP provides, manages and authorizes most of your health care services. BCN offers comprehensive coverage with no deductible and affordable copays. Health education, disease management, care management and pharmacy programs help you get healthy and stay healthy. Healthy Blue LivingSM
You are rewarded for adopting and maintaining a healthier lifestyle with Healthy Blue Living. You automatically receive lower copays and deductibles for the first 90 days of coverage. To remain eligible for the lower cost-sharing, you and your spouse need to complete a health risk appraisal, meet with your primary care physician to complete a wellness commitment form and adopt a healthy lifestyle as recommended by your primary care physician. Traditional
Blue Managed Traditional, also referred to as “fee-for-service” program, offers you the freedom to receive care from the health care provider of your choice. Blue Managed Traditional covers basic hospital and surgical coverage, with a focus on treating illnesses and injuries versus preventive and wellness care. Our Traditional product includes managed care features like utilization review and quality assessment to ensure that care is medically necessary and appropriate. When you receive covered services from participating providers, you will only have to pay for your deductible and copayment.
