Plan options include:

• Blue Vision 12: Exam, lenses and frames every 12 months
• Blue Vision 24: Exam, lenses and frames every 24 months
• Blue Vision — Exam Only: Available to ASC and ERS groups
• Blue Vision — Exam Plus Allowance: Available to ASC groups
• Blue Vision Choice: Voluntary product, available to underwritten groups with 2-99 contracts

• VSP has a network of 1,400 providers in Michigan and over 33,000 providers nationwide
• VSP does all servicing, network management and claims processing
